Description

Joint Commissioning of Community Based Domiciliary Care and Support in Leicestershire County - The Help to Live At Home Service (HTLAH) Leicestershire County Council, working jointly with West Leicestershire Clinical Commissioning Group and East Leicestershire and Rutland Clinical Commissioning Group intend to commission new home-based care and support services to meet the health and social care needs of service users and carers across the county of Leicestershire. Service users will be individuals aged over 18 who have been assessed as eligible to receive services commissioned either through the local authority or assessed by health under continuing healthcare (CHC). One of the commissioners’ key strategic intentions is to support people to live independently in their own homes for as long as possible. In order to do this, the commissioners require a stable and robust market of providers who have an experienced, motivated and well-trained workforce who are able to deliver a high volume of affordable, consistent and high quality services. The requirement includes a wide range of flexible services for people with varying levels of need which will include the delivery of associated social care and healthcare tasks. The aims of the service are to enable service users: - to safely remain in their own home; - to achieve their maximum potential through delivery of “step up” community reablement service; - to maintain their independence by ensuring that reablement remains central to service delivery. The service will also facilitate people’s safe and timely discharge from hospital. The final commissioning model will be determined upon completion of an ongoing market engagement process; the next stage of which is scheduled to focus on supporting market readiness in the independent sector. At previous events you have helped us to explore strategic options relating to: Delivery models; geographical options and outcome based commissioning . Reablement in practice; Assistive Technology; Social Capital and developing community resources. The main aim of this next round of co-production is to provide information about the approach we intend to take with regard to commissioning the new service. We want to draw on the strategic expertise in your organisations to assess the benefits and risks of the chosen approach from your perspective. These events will be of particular relevance to Company Owners, Directors and Business Development Managers. The events are planned for: . Thursday 30th July- 12.00 to 3.30 and . Wednesday 5th August- 9.00 to 12.30 Both events will be the same so you need only attend one, and will be held at the: NSPCC National Training Centre 3 Gilmour Close Leicester LE4 1EZ The agenda and details of the approach we intend to take will be circulated ahead of the meeting, but please take this opportunity to confirm you attendance by emailing your Organisation, Name and Position to htlah@leics.gov.uk and we will reserve a place for you. Thank-you very much for your support to date, we hope you can make the time to support us again at this critical point in the development of the Help to Live at Home service.
